The form below has been created to allow students to verify their intentions for UNA-Orlando internship. It happens often that students will request internships with multiple organizations and eventually choose one organization over the others. With this in mind, the Student Verification form has been designed to allow students the opportunity to declare their current intentions while maintaining the option to change their mind later to reflect changing circumstances.

You can complete the form at any time and revise it later as is necessary. Reasons to revise the agreement include:

• to reaffirm the student's continued interest in pursuing a UNA-Orlando internship

• to report that a decision has been made to choose another option, meaning that the student has decided that she/he will no longer pursue a UNA-Orlando internship

• to alter a decision to pursue an Academic Internship or a Non-Academic Internship (see notes below describing the difference between Academic and Non-Academic internships)
